Navigating the programs and services available for individuals with disabilities can be daunting. NFHF has provided several resources for the most common state and federal programs available.
MEDICAID
Do I Qualify?
- Receive Supplemental Security Income (SSI) from the Social Security Administration (SSA)
- Get financial help from the Office of Family Support (OFS) through the Family Independence Temporary Assistance Program (FITAP)
- Are disabled according to the Social Security Administration’s definition
- Have corrected vision no better than 20/200
- Are a child under age 19
- Are pregnant
- Have no insurance and need treatment for breast and/or cervical cancer
- Receive Medicare coverage and are low-income, or
- Are aged 19 to 64 years old, have a household income less than 138% of the federal poverty level, doesn't already qualify for Medicaid or Medicare, and meet citizenship requirement

How Do I Apply for Medicaid?
- Apply online through Louisiana Medicaid Online Application (OLA)
- Call the Medicaid Hotline: 1.888.342.6207
- Contact a Northshore Area Application Center
- Download and print a Medicaid application and then mail or fax it to Medicaid as directed on the form.
- Contact your Local Medicaid Office
Social Security
Who Qualifies for Social Security Disability Benefits?
- "Social Security pays benefits to people who can’t work because they have a medical condition that’s expected to last at least one year or result in death."
- "Certain family members of disabled workers can also receive money from Social Security."

What is Supplemental Security Income (SSI)?
- "pays benefits to disabled adults and children who have limited income and resources."
What is Supplemental Security Disability Income (SSDI)?
- "pays benefits to you and certain family members if you are “insured,” meaning that you worked long enough – and recently enough - and paid Social Security taxes on your earnings."

OFFICE FOR CITIZENS WITH DEVELOPMENTAL DISABILITIES
The Office for Citizens with Developmental Disabilities (OCDD) serves as the Single Point of Entry (SPOE) into the developmental disabilities services system and oversees public and private residential services and other services for people with developmental disabilities.
FLORIDA PARISHES HUMAN SERVICE AUTHORITY
Florida Parishes Human Services Authority (FPHSA) is the single point of entry for developmental disabilities services in the following parishes: Livingston, St. Helena, St. Tammany, Tangipahoa, and Washington Parish.
